Medi-Cal can cover bariatric surgery, including band removal and revision, when its written criteria are met and the request is approved in advance. In San Bernardino and Riverside counties, most Medi-Cal members belong to a managed care plan such as IEHP, which reviews requests through your primary care doctor and medical group. The rules below are as of October 2026; verify with your plan.
Which Medi-Cal plans serve the Inland Empire
The California Department of Health Care Services (DHCS) runs both counties under a "Two-Plan" model. Its county table for 2024 lists two plans in Riverside and San Bernardino counties:
- Inland Empire Health Plan (IEHP), which describes itself as a public, not-for-profit plan
- Molina Healthcare of California
DHCS also lists Kaiser Permanente in both counties for certain members, mainly people who were recently Kaiser members or have a family member in Kaiser. Your plan's name is on your Medi-Cal plan ID card.
Medi-Cal's written criteria for bariatric surgery
The Medi-Cal provider manual says surgery for severe obesity needs an approved Treatment Authorization Request (TAR), meaning prior approval. The request must document all of the following:
- a body mass index (BMI) over 40, or over 35 with a serious related condition such as sleep apnea, uncontrolled diabetes, or serious heart, lung, nerve or joint problems likely to improve with surgery
- failure to keep weight off with conservative treatment, for example severe obesity for at least five years despite a physician-supervised program of at least six months, or a documented two-year managed weight-loss program
- a clear understanding of the alternatives and of how life changes after surgery, including the risks
- a pre-operative medical consultation showing you are an acceptable surgical candidate
- no contraindications, including an alcohol or substance use problem in the last six months or severe psychiatric impairment
- a treatment plan with dietary evaluation, nutrition counseling, exercise counseling and psychological support before and after surgery
These are Medi-Cal's coverage rules. They are older and stricter than the 2022 clinical guidelines described on the candidacy page.
Band removal and revision under Medi-Cal
The same manual lists band revision, band removal, band removal and replacement, and port repair or removal among the procedures that need a TAR. It says repeat surgery or revision may be medically necessary to correct complications, including implanted device failure, a pouch of the wrong size, stricture, fistula or obstruction.
A repeat operation requested because weight loss was not achieved or kept off must include documentation that you took part in, and followed, the earlier post-operative program. Records of your fills, clinic visits and nutrition follow-up help here. Our page on insurance for band removal and revision compares this with commercial policies.
How approval works with IEHP
IEHP's 2026 Medi-Cal handbook explains that your primary care provider (PCP) belongs to a medical group, which directs your care. In most cases you see specialists in the same medical group, and your PCP asks for prior authorization to see a provider outside it. Clinical staff review requests.
- Standard requests: a decision within five business days of receiving the information needed, and no more than seven calendar days from the request.
- Urgent requests: within 72 hours when waiting could seriously endanger your health.
- More information needed: up to 14 more calendar days.
If IEHP denies, delays or changes a request, it sends a Notice of Action. The handbook says you have 60 days from that notice to appeal. After an appeal, you can ask for a State Hearing and an Independent Medical Review with the Department of Managed Health Care. IEHP also covers second opinions; see our page on getting a second opinion.
GLP-1 medications on Medi-Cal
Medi-Cal Rx stopped covering GLP-1 drugs for weight loss on January 1, 2026. Zepbound and Saxenda were removed from its drug list, Wegovy is no longer covered for weight loss, and Ozempic, Mounjaro and similar drugs remain limited to type 2 diabetes and other non-weight-loss uses. Requests for members under 21 are still reviewed for medical necessity. See GLP-1 medications with a Lap-Band.
Frequently asked questions
Does Medi-Cal cover Lap-Band removal?
It can. The Medi-Cal provider manual lists band removal among the procedures that need prior approval, and says revision may be medically necessary to correct complications such as device failure, stricture or obstruction. Approval depends on documentation, so imaging reports and clinic notes describing your symptoms are important. IEHP members go through their PCP and medical group.
Can I choose a bariatric surgeon outside my IEHP medical group?
Sometimes. IEHP's handbook says that when a service is not available in your medical group, your PCP can refer you elsewhere with prior authorization. Outside emergencies, care outside your medical group generally needs approval first. Ask your PCP, or call IEHP Member Services at 1-800-440-4347, before booking with a surgeon outside your group.
What should I do if IEHP denies my surgery request?
Read the Notice of Action for the reason. If paperwork was missing, your doctor may be able to resubmit. You can appeal within 60 days by phone, mail or online. If IEHP keeps the denial, or does not decide within 30 days, you can ask for a State Hearing and an Independent Medical Review.
Does IEHP cover bariatric surgery for adults with Medicare and Medi-Cal?
IEHP's handbook says Medi-Cal is the payer of last resort, so Medicare pays first for services it covers, and that members with both can use providers in either coverage. Medicare's own bariatric rules then apply. See Medicare and Lap-Band care, and call your plan to confirm.
