Gastric sleeve care for Upland and Claremont residents
Upland borders Rancho Cucamonga on the west, and Claremont lies just beyond Upland, across the Los Angeles County line. Whichever one you live in, a first talk about a gastric sleeve with Dr. David G. Davtyan, MD, FACS, FICS, FASMBS, takes place at the Red Oak Street office. The detail that needs attention is insurance, because the two cities are in different counties.
East along Foothill or Arrow
Foothill Boulevard runs east through Claremont and Upland into Rancho Cucamonga; from there, Aspen Street leads south to Red Oak Street. Farther south, Arrow Route is the road Red Oak Street meets. OpenStreetMap labels this corridor Arrow Route or Arrow Highway depending on the stretch, so look for both names. From the north side of either city, the 210 east to Haven Avenue, then south to Foothill Boulevard, also works. Confirm the route in a map app.
The address is 8330 Red Oak Street, Suite 201, Rancho Cucamonga. Surgery is done at a hospital where Dr. Davtyan has privileges; ask the office which one.
Two cities, two profiles
January 1, 2026 population estimates from the California Department of Finance are 78,406 for Upland and 38,262 for Claremont.
CDC PLACES models health measures city by city. Its crude estimates for adults, based on 2023 data, are:
| Adults, 2023 | Upland | Claremont |
|---|---|---|
| Obesity | 30.1% | 20.7% |
| Arthritis | 22.6% | 19.2% |
| High blood pressure | 31.4% | 26.3% |
| No leisure-time physical activity | 21.6% | 17.2% |
Crude estimates are not adjusted for age, and the arthritis share is not unusual for the area. Roughly one adult in five reports it here, and for many people painful joints are part of why weight is hard to lose. The next section looks at what the guidelines say on joints and age.
Joints, age and candidacy
The 2022 guidelines from the American Society for Metabolic and Bariatric Surgery (ASMBS) and the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) recommend surgery at a body mass index (BMI) of 35 or more and say it should be considered at 30 to 34.9 with metabolic disease.
On joints, the same guidelines note that some orthopedic societies discourage hip and knee replacement at a BMI above 40, and that some reports suggest bariatric surgery may work as a bridge to joint replacement. On age, they state that there is no upper age limit, and they advise careful assessment of other health conditions and frailty. They also report slightly higher complication rates in people in their seventies. Our page on a gastric sleeve after age 60 goes through the evidence, and comparing weight-loss options sets surgery beside non-surgical choices.
The county line and your plan
The California Department of Health Care Services (DHCS) organizes Medi-Cal managed care by county, and its 2026 table gives different plans on each side of the line:
| City | County | Plans in the 2026 DHCS table |
|---|---|---|
| Upland | San Bernardino | Molina Healthcare of California, Inland Empire Health Plan, Kaiser Permanente, SCAN Connections |
| Claremont | Los Angeles | Health Net Community Solutions, L.A. Care Health Plan, Kaiser Permanente, SCAN Connections |
DHCS describes SCAN Connections as a Medicare Advantage special needs plan covering both Medicare and Medi-Cal benefits. The office is in San Bernardino County, so if you live in Claremont and have Medi-Cal, call member services and ask whether your plan allows care with a surgeon in another county. Do not assume either way.
The practice works with PPO and HMO plans that allow care with Dr. Davtyan. For anything else, ask us whether we can see patients on your plan. Medicare's national coverage rules do not change at the county line, though Medicare Advantage plans have their own networks; see Medicare rules for bariatric care and insurance requirements for a sleeve.
What living close makes easier
A sleeve is one operation followed by years of check-ins. The ERAS Society's 2021 bariatric guideline calls for supplements and nutritional blood tests for life. A shorter trip can make visits easier to keep, including for new heartburn, food sticking or a weight stall. See the follow-up schedule for what those visits tend to cover.
Closeness has one limit. For signs of an emergency, which include not keeping any liquid down, blood in vomit, black stools, severe pain in the chest or abdomen, a fever with a fast pulse or trouble breathing in the early weeks, or one swollen and painful calf, use 911 or the emergency room nearest your home. Call the office afterward.
Frequently asked questions
I live in Claremont and have a PPO. Does the county line matter for me?
Probably much less. The county-by-county list applies to Medi-Cal managed care. A PPO decides coverage by its own network and benefits. Have your card ready when you call, and ask whether your plan allows care with Dr. Davtyan. Also call your insurer directly to ask what approval a sleeve needs under your policy.
My knees are the main reason I want to lose weight. Is that a reason for a sleeve?
It can be part of the picture. Under the 2022 ASMBS and IFSO guidelines, candidacy rests mainly on BMI, with related conditions weighed alongside it. If an orthopedic surgeon has asked you to lose weight before a joint replacement, bring those notes. No operation can promise a particular result for joint pain.
How many times would I come to Rancho Cucamonga in the first year?
Ask the office for its schedule, since it depends on your surgery and your health. The National Institute of Diabetes and Digestive and Kidney Diseases says the surgeon will tell you what type of follow-up you need. Before the operation there are also consultation and evaluation visits. Ask whether any of them can be done by phone or video.
Can I talk with Dr. Davtyan about medication instead of surgery?
Ask the office whether a medical weight loss program with GLP-1 medications is currently available. Booking a consultation does not commit you to an operation, and it is reasonable to ask how surgery compares with other options for you. Our weight-loss medication page has background on these medicines.
