Gastric sleeve care for Ontario, CA residents
If you live in Ontario and are considering a gastric sleeve, you can meet Dr. David G. Davtyan, MD, FACS, FICS, FASMBS, at the Red Oak Street office in Rancho Cucamonga, the city just north of Ontario. The first consultation is complimentary. The operation itself is done at a hospital where he has surgical privileges, so ask the office which one.
From Ontario to Red Oak Street
The office is at 8330 Red Oak Street, Suite 201, Rancho Cucamonga. Red Oak Street meets Arrow Route a short way east of Haven Avenue. From the south side of Ontario, Haven Avenue runs north across the 60 and the I-10 to Arrow Route; turn east there. From central Ontario, Vineyard Avenue runs north to Arrow Route. These come from OpenStreetMap road data, so check the route in a map app before you leave.
Ontario's health figures and what they do and do not tell you
The California Department of Finance put Ontario's population at 187,863 on January 1, 2026. CDC PLACES publishes modeled health estimates for each city. These are its crude estimates for adults, based on 2023 data, set beside Rancho Cucamonga:
| Adults, 2023 | Ontario | Rancho Cucamonga |
|---|---|---|
| Obesity | 33.6% | 30.0% |
| Diagnosed diabetes | 12.0% | 10.5% |
| Food insecurity in the past year | 25.8% | 15.6% |
| No health insurance, ages 18 to 64 | 17.0% | 8.8% |
| Lack of reliable transportation in the past year | 12.6% | 8.4% |
These numbers describe a city, not you. They do point to three practical questions worth settling early: how diabetes affects the decision, whether your food budget can support life after a sleeve, and how you will pay for care and get to it.
If diabetes is part of your reason
The 2022 guidelines from the American Society for Metabolic and Bariatric Surgery (ASMBS) and the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) recommend surgery for a body mass index (BMI) of 35 or more, and say it should be considered at a BMI of 30 to 34.9 with metabolic disease such as type 2 diabetes. Insurance plans may still use older, stricter rules. Bring your recent lab results and medication list to the consultation. Our pages on gastric sleeve and type 2 diabetes and sleeve candidates go further.
Paying for food, vitamins and care
Under the 2021 Enhanced Recovery After Surgery (ERAS) Society guideline, patients take vitamins and minerals for life and have their blood levels monitored. If the grocery budget is tight, say so at your consultation, because the eating plan has to fit what you can buy.
Ontario is in San Bernardino County. The California Department of Health Care Services (DHCS) lists the county under its Two-Plan model. Its 2026 table names Molina Healthcare of California, Inland Empire Health Plan (IEHP) and Kaiser Permanente, along with SCAN Connections, a Medicare Advantage special needs plan for people who have both Medicare and Medi-Cal. Those are simply the county's plans. Each one decides which surgeons its members may see.
The practice works with PPO and HMO plans that allow care with Dr. Davtyan. For any other plan, ask us whether we can see patients on your plan. How Medi-Cal plans approach bariatric surgery is explained in IEHP, Medi-Cal and bariatric surgery, and general plan rules in insurance requirements for a sleeve.
IEHP's Medi-Cal page says the plan provides no-cost, round-trip rides to medical specialists, arranged by the member through IEHP Transportation. If you have a different plan, ask its member services what transportation help exists.
Planning the weeks around surgery
The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) says that before weight-loss surgery you will meet several professionals, such as an internist, a dietitian, a psychiatrist or psychologist, and a bariatric surgeon. You cannot drive yourself home from the hospital, and you should not drive while taking opioid pain medicine, so line up a driver early. Follow-up continues long after the first months; our follow-up schedule page explains what is usually checked, and your surgeon sets your own timetable.
If something goes wrong at home
Living one city away does not make the office the place to go in an emergency. If you cannot keep liquids down, vomit blood, pass black stools, have severe chest or abdominal pain, develop a fever with a racing heart or shortness of breath in the first weeks, or have a painful, swollen calf, call 911 or go to the nearest emergency room. Tell the staff you have had a sleeve gastrectomy and when. For non-urgent questions, call the office.
Frequently asked questions
Where would I go for visits if I live in Ontario?
Office visits take place at 8330 Red Oak Street, Suite 201, in Rancho Cucamonga. The operation does not happen there. When you call, ask where parking is and which appointment times are open, and check the route in a map app, since the road you want depends on which part of Ontario you start from.
Does the sleeve operation take place at the Red Oak Street office?
No. A sleeve gastrectomy is done under general anesthesia in a hospital operating room. Dr. Davtyan operates at a hospital where he has surgical privileges. Ask the office which hospital it will be, so you can plan the trip to that address and not to Red Oak Street. Your driver needs the same address.
I work nights or rotating shifts. Can I fit in the pre-surgery steps?
That depends on your schedule and on what your plan requires. Mention your shift pattern on the first call, and ask whether visits can be grouped and which times are available. Tests can sometimes be done near home, depending on the plan, so ask before booking. Our page on returning to work covers lifting jobs after surgery.
My diabetes is the main reason I am looking at surgery. Does that count?
It can. The 2022 ASMBS and IFSO guidelines say surgery should be considered for people with metabolic disease at a BMI of 30 to 34.9, and recommend it at 35 or more. Nobody can promise what will happen to your diabetes. Medicines must be adjusted by your prescriber, not stopped on your own.
